Shockingly, HBO's television series Game of Thrones featured a severed head that resembled former President George W. Bush. Showrunners for the series say it wasn't intentional.

I normally give people the benefit of doubt, but come on, folks. Producers and others edit, re-edit, and view for content. If it was a joke, it was in very poor taste. They claim it wasn't "a political statement." Really?

The executives of HBO have edited the material and apologized, but the head's out of the bag, so to speak.
